Personal Care Assistant

We are seeking dedicated and compassionate Personal (Care) Assistants to provide essential support to individuals in need of assistance with daily living activities. This role is ideal for someone who is passionate about patient care and has experience in long-term care settings. The Personal Assistant will play a crucial role in enhancing the quality of life for clients by ensuring their needs are met with dignity and respect.

Duties

  • Assist clients with daily living activities, including personal hygiene, dressing, and grooming.
  • Provide companionship and emotional support to clients, fostering a positive environment.
  • Monitor client health by observing changes in condition and reporting any concerns to healthcare professionals.
  • Maintains accurate and current documentation records via PHS (Practical Health System) and/or Sandata system.
  • Document and submit daily notes and other documentation in a timely manner.
  • Attend required in-services, staff meetings, COS, pre-planning, and Annual ISP meetings.
  • Help clients with mobility and transportation needs, ensuring safe travel to appointments or social activities.
  • Maintain a clean and safe living environment for clients, including light housekeeping tasks.
  • Uphold resident rights and ensure that all interactions are conducted with respect and confidentiality.
  • Support clients in memory care or home health settings by engaging them in meaningful activities.
  • Be responsible for implementing and adhering to all ISP/PCSP action/outcome steps, Behavioral Support Plans, Physical Therapy Plans, Occupational Therapy Plans, O&M, and Speech Therapy Plans.
  • Follows Reportable Event Management Policy, Abuse Prevention, and Safety in the Home Guidelines, Agency Call-Out Procedure, DDA/MCOs rules and regulations regulations and training requirements, and Emergency Protocols at all times.
  • Follows infection control guidelines at all times.
  • Performs other duties as assigned.

Qualifications

  • Must be at least (18) years of age.
  • High school diploma or GED required.
  • Experience with Sandata is preferred.
  • Medication certification as active is preferred.
  • A valid TN drivers license, proof of current automobile insurance, and acceptable driving record are required.
  • Must pass the criminal background check, TN Abuse Registry, TN Sexual Offender Registry, TN Felony Offender List, SIRI, LEIE, SAMS, and any other Registry Checks including previous employer, and personal reference checks.
  • Strong interpersonal skills with a patient and empathetic approach to caregiving.
  • Ability to work independently as well as part of a team, demonstrating reliability and professionalism.
  • Previous experience in supported living, assisted living, personal assistance, home health care is a plus.

Pay: $16.75 per hour

Expected hours: 10 – 40 hours per week (flexible schedules available)
