Social Worker - Behavioral Health Case Manager/Clubhouse Staff

Under general supervision of the Human Services Supervisor – Behavioral Health, this position supports the mission of Blue Earth County Human Services by effectively and efficiently delivering essential services to families and individuals so they can achieve their potential through active community participation. The Social Worker – Behavioral Health Case Manager will be responsible for providing Adult Mental Health Targeted Case Management (TCM) Services to adults with serious and persistent mental illness and to adults under court commitment as mentally ill or chemically dependent. This position is responsible for assessing, accessing, referring, coordinating, monitoring, and planning mental health services. This position also monitors court commitments, participates in discharge planning, provides crisis assistance, and participates in professional staff development.  In addition, this position plans and facilitates clubhouse programs and activities, mentors clubhouse members and volunteers, and ensures a safe and secure environment for clubhouse members. Assists clubhouse members to interact positively and to participate in the daily operations of the clubhouse program.
