Home Health Aide - Ashburn

Responsibilities

  • Assist special needs patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, toileting, and mobility support using tools like Hoyer lifts when necessary
  • Monitor patient health by observing vital signs, reporting changes, and maintaining accurate medical documentation within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems
  • Administer medications and perform catheter care following prescribed instructions while adhering to HIPAA regulations for patient privacy
  • Support patients with memory care needs related to dementia or Alzheimer's disease through specialized techniques in memory care and dementia care strategies
  • Assist with meal preparation, feeding, light cleaning, laundry, and maintaining a safe and clean environment in compliance with health standards
  • Document patient progress accurately in medical records and communicate effectively with healthcare teams regarding patient status or concerns
  • Provide compassionate hospice care when applicable, ensuring comfort and dignity during end-of-life stages

Qualifications

  • Proven experience in home health care, assisted living facilities, nursing homes, or long-term care environments
  • Strong knowledge of medical terminology, medication administration, vital signs monitoring, and infection control protocols
  • Hands-on experience working with individuals with developmental disabilities or disabilities requiring specialized support
  • Certification in CPR (Cardiopulmonary Resuscitation) and training in hospice or Alzheimer’s care preferred
  • Ability to operate medical equipment such as Hoyer lifts and perform catheter care safely and effectively
  • Familiarity with EMR/EHR systems for accurate documentation; experience with home & community care is highly valued
  • Excellent patient observation skills combined with a caring attitude to ensure safety and well-being at all times
